Student Withdrawal Form
Please let us know if there is anything we can do to improve our services. All of our instructors are available to meet with parents to discuss any issues students may be having (i.e. not wanting to practice, class structure, progress). If you need to withdraw for any reason, you are required to give 4 weeks written notice. Tuition is due during those 4 weeks. Billing will continue until written notice is received.
